How Can We Help You?

Entry into our Adult Services Program is done through our Support Centers Monday through Friday from 9 a.m.- 3 p.m.

Interim Housing

Breakthrough's Interim Housing Program is designed to provide transitional shelter for up to 120 days while supporting individuals in attaining stability.

Our Intake Process begins before space is generally available in our program. In an effort to provide holistic services, our initial entry into the program involves planning and needs assessment with our staff. This individual session is incorporated into our Intake Process and allows us to assist with many immediate needs while Participants wait for a bed. Needs such as clothing, laundry services and meals become available after Intake has been completed. Prior to entering into the overnight program you will be given the opportunity to create an Individual Service Plan outlining your goals and action plan to reach them.

Once you have been granted overnight access and the 120 days begins you have access to a Case Manager and our Life Transforming Skills Team that consists of our Housing, Behavioral Health and Employment Coordinators. Through individual support and links to other resources you receive more assistance in accomplishing the goals contained in the Individual Service Plan. We value partnerships and collaborations thus we make every effort to broaden the network of individuals you have personal access to by encouraging participation in all activities available.
